Rural Alliance Care Community Team is an integrated service supporting the population attached to the 6 GP practices, we are base at Chowley Oak Tattenhall and have some office space at Tarporley War Memorial Hospital.

The team consists of a Team Manager, Care Coordinator, Community Nurse, Physiotherapist, Occupational Therapists, Therapy assistants. We work very closely with our local GP’s,  Cheshire West and Chester social Services and all the hospitals within the area.

The  community nursing team offers nursing services to the housebound population of the locality and takes referrals from GP’s, hospitals, carers and self referrals.

The therapists also work with patients in their own homes to maintain and promote independence.

We have been made aware that there are a rising number of cases of measles in Cheshire West.

If your child is unwell with a rash and fever and needs to be seen, you may be asked to wait outside your GP surgery prior to their appointment.

If your child has missed an MMR vaccination or they have not been vaccinated please contact your GP reception to book an appointment. It’s never too late.
